Job description

  • Job Summary
    • Under general supervision of the Leader of Home Health / Hospice and Home Health Clinical Care Coordinator and in accordance with departmental policies and professional standards, prepares social histories, provides social casework to patients and families having difficulty in social functioning primarily as it affects their health, refers and aids in the referral process to other agencies, supervises social work assistant, and generally serves as a social work consultant in the HHC department's total program. Direct social services are provided to HHC patients only upon receipt of a physician's order.
  • Job Specifications
    • EDUCATION AND EXPERIENCE: Masters degree from a school of social work accredited by the Council on Social Work Education. At least one (1) year of social work experience in a health care setting. Must be an eligible candidate for or hold a valid State of Illinois Social Worker License (LSW or LCSW). Must maintain valid driver's license if applicable.

    • JOB KNOWLEDGE: Knowledge of all social work methods with particular skill in casework and the ability to adapt this knowledge and skill to the Home Health setting. Some knowledge and understanding of the principles of public health administration and practice. Expertise and ability in family assessment. The patient population must be considered when assessing job knowledge required from the employee.

    • ABILITIES, INTERESTS AND TEMPERAMENTS
      • Ability and desire to establish a good working relationship with other Home Health Care team members as well as other hospital members.
      • Ability to collaborate effectively
      • Successfully completes the Home Health Care orientation, ongoing hospital wide CBLs (computerized based learning), participates in departmental inservices and demonstrates competency by completing the Home Health Care specific competency testing/evaluation program, as appropriate to the Social Worker role. Continuing education programs are designed to augment the staff's knowledge of pertinent developments in patient care and to foster a performance improvement environment.
      • Must have the ability to work with a variety of patients of all populations (e.g. developmentally appropriate care, cultural diversity, patients with pain, victims of abuse or neglect, and diagnosis or procedure specific such as patient with congestive heart failure or cesarean delivery).
