Social Worker W/ Remote Work Option

Citizens Memorial Hospital (CMH) is recruiting a Licensed Social Worker, Licensed Professional Counselor, or an individual pursuing a related license to provide counseling to patients for behavioral change.

This position requires completion of MSW degree and LCSW license, a Master's in Counseling with a LPC license, or a MSW degree with a LMSW license.

The counselor helps the patient to identify barriers and behaviors that lead to poor outcomes and, through a series of brief visits, to develop goals and strategies or behavioral modifications to help them reach their goals. The counselor is also responsible for the assessment and transition of care for patients in the Emergency Department and from Primary Care into Therapy. You will work with a set of clinics, providers, and nurse care managers to coordinate and provide care. You will work with patients for 3-5 visits, including in-office visits when necessary. This position offers variable work in the clinic setting, flexibility in scheduling, and an evening/weekend call rotation.

This position is full-time, and a Remote Work Option is available for residents within the commuting range of Bolivar as on-site visits may be occasionally needed.
