Job description

Central Ohio Primary Care Physicians are seeking a Social Worker Supervisor to lead our Comprehensive Care team located in Westerville, OH. The Social Worker Supervisor manages an ongoing caseload in addition to leading the social work team. Together with the manager, the role provides guidance on departmental and organizational policies and procedures, clinical consultation, and problem-solving to ensure patients receive timely, effective and high-quality care. They are the subject matter expert in his/her discipline and will always strive for clinical excellence, mindful service, and the modernization of primary care. This is a full time role working first shift hours Monday- Friday. Opportunity to be hybrid upon successful completion of training.

POSITION(S) SUPERVISED: Licensed Social Workers, Pediatric Social Work Lead, Community Health Workers

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Lead clinical huddles for collaboration of workload and case discussions 
  • Work in collaboration with the Clinical Educator to support staff training and educational needs.
  • Coordinate in-service training, workshops, and re-training for social work staff.
  • Monitor caseloads and incoming referrals and adjusts workload as necessary 
  • Serve as a resource to team on complex issues.  
  • Provide support for planned/unplanned absences.  
  • Select and hire new social work candidates.
  • Serve as a mentor of the social work team within COPC practices. Collaborate with practices to understand care management and assist in relationship building as needed with the assigned Social Worker.
  • Work to elevate staff daily to be the best for our patients, our physicians and for each other.
  • Set the highest standard of what is expected of our Social Workers and lead by example.
  • Maintain reduced patient caseload   and provide coverage for existing caseloads within the social work team as needed.
  • Oversees the distribution of work for the social work team to ensure a balance among the staff.
  • Conducts quarterly audits on social work staff to ensure the delivery of patient care is timely and safe. Assign file audits to social work staff to meet this requirement.
  • Maintain working knowledge of all social work processes, workflows, and policies.
  • Ensure social work team adheres to organizational policies and department standards Together with the manager, participate in the development of individual performance improvement plans to help retrain and retain existing staff.
  • Stay updated on current trends in the healthcare industry and develop relationships with community-based organizations to partner for patient care needs.
  • Provide coaching and mentoring to the social work team.
  • Conduct 90- day and annual performance evaluations in conjunction with department manager.
  • Perform miscellaneous job-related duties as assigned.

 

QUALIFICATIONS:

Education, Licensures & Certifications

  • Required: Social Worker licensed to practice in the state of Ohio. Must maintain professional licensure for continued employment.
  • Required: Minimum of 3-5 years of care management experience in acute care, home healthcare, hospice, managed care or community behavioral setting
  • Preferred: Prior clinical education training or leadership experience; Master's in social work

Knowledge, Skills & Abilities

  • Thorough knowledge and understanding of clinical protocol, procedures, and standards within area of care management and social work acumen.
  • Change agent for the social work team
  • Must be detail-oriented and highly organized
  • Self-disciplined, energetic, passionate, and innovative
  • Knowledge of related care management standards (CMSA).
  • Ability to provide operational guidance and leadership to care management staff.
  • Ability to assess educational needs and design and develop responsible curricula.
  • Ability to plan, implement, evaluate, and revise the training program to ensure learning objectives are met.
  • Ability to work in stressful and ambiguous situations.
  • Technical leadership skills with EHR and clinical documentation platforms.
  • Advanced skills and knowledge in care management areas including but not limited to legal, ethical, and social aspects.
  • Highly developed verbal and written communication skills and the ability to present effectively to small and large groups.
  • Strong interpersonal skills and ability to work effectively at all levels in a collaborative team environment.
  • Requires the ability to communicate with patients, office personnel, and the community.
  • Requires a proficient driving record to travel to visit patients is necessary.
