Manages HIV Medical & Non- Medical Case Management Program. Supervises, coordinates and directs medical case management & non medical case management services of the District Ryan White HIV Program.
All Coastal Health District County Health Department employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.
Job Responsibilities
1. Supervises staff which involves hiring decisions, employee orientation, assignment of work duties, approval of work schedules/leave request and performance evaluations.
2. Maintains program specific Medical Case Management SOPs consistent with Part B, C
& D site visit requirements at all times. Performs annual SOP review/revision to ensure
approved SOPs reflect actual operating practices and obtains annual administrative
signatures.
3. Effectively manages systems to achieve and maintain evolving consumer linkage and
retention objectives set forth in the NHAS goals and objectives.
4. Effectively manages systems that maintain consumer Ryan White, ADAP and HICP
eligibility documentation consistent with Ryan White and State guidance as evidenced by
successful site visits.
5. Maintains program specific NON-Medical Case Management SOPs consistent with Part
B, C & D site visit requirements at all times. Performs annual SOP review/revision to
ensure approved SOPs reflect actual operating practices and obtains annual
administrative signatures.
6. Manages Client Advocacy activities for the program
7. Manages the District HIV Program case management Quality Assurance
Minimum Qualifications
Master's degree from an accredited college or university AND licensure AND Three years of related experience, One year of which in a supervisory role OR Master's degree from an accredited college or university AND Three years of experience at the lower level Social Worker, NonLicensed 3 (HCP122) or position equivalent.
