What are the primary job duties?
Provides behavioral skill development, case management, and consultation services, which includes;
- Implements and monitors individualized treatment plans for intensive in-home therapy clients as directed by lead in-home therapist.
- Provides coordination to community agencies for SED children and families as needed.
- Records clinical treatment notes and progress notes.
- Prepares and presents client cases for staffing with primary therapist and physicians.
- Provides client transportation when appropriate.
What are the minimum qualifications?
- Bachelor degree in social work or a related human service field required.
- One year experience within the last seven years working with the mentally ill required, experience with children's mental health preferred.
- Certification or eligible for certification as a Social Worker in Wisconsin preferred.
