SalaryThe incumbent will perform the following duties:
- Facilitate groups and/or one-on-one sessions in which you plan, coordinate, manage and implement Psycho-Educational lesson plans to help clients deal with difficulties and overcome dependencies
- Provide services to consumers concerning their Active Treatment; Include and not limited to classes, activities, note writing, counseling and individual therapy
- Provide necessary weekend coverage and support for peers and staff
- Participate in other special Events and assignments through the hospital
- Under general supervision, provide a broad range of social services to consumers and their families
- Participate and collaborate on Treatment Teams
- Liaison with community resources
- Interview service users and assess their current condition, needs, strengths and weaknesses
- Address each case as a unit and set tailored measurable goals
- Monitor and evaluate clients' progress and provide feedback to the team to modify treatment plans accordingly
- Offer information and feedback to the team on the best course of action during sessions
- maintain accurate and timely documentation and report on clients' status
- Act as a key-worker and cooperate with multidisciplinary terms
- Refer clients to community services to help them in recovery
This is temporary or hourly paid work performing professional social/counseling services. Positions in this job are established for less than 9 months or scheduled for less than 30 hours per week and are not eligible for benefits if the position status is temporary (i.e., SCOA 513). Positions in this job are established for greater than 9 months and scheduled for greater than 30 hours per week if the position status is permanent labor (i.e., SCOA 512). Normally permanent labor positions are eligible for Health Insurance benefits.
