Job Description The Caring Transitions Team is comprised of Social Workers, Peer and Family Advocates, a Transitions Coordinator, and a Medical Director, who will support children, youth, and their families who present at H+H hospitals in Queens following a suicide attempt or serious suicidal behavior, from point of entry until 90 days post-discharge.
The Caring Transitions Team will engage with youth and families upon presentation to the ED or direct admission. If interested in participating, the team will follow them throughout the hospital course, participating in aftercare planning. Upon discharge the team ensure that youth are connected to outpatient treatment, provide caregivers with tools to understand and identify risk factors and warning signs of suicide, assist the youth in developing life-affirming goals, and develop the capacity of other trusted adults and community supports in the lives of the youth and families they serve.
In certain cases, the team will provide bridging care/treatment until the youth is connected to outpatient services. Duties & Responsibilities The Social WorkerLevel V is an integral part of this interdisciplinary team. This position will be responsible for supervising the team.
This combines significant clinical assessment and brief treatment with a knowledge of city systems, individual and team supervision. This position oversees the work of the team and will also provide direct services to families in emergency, inpatient, outpatient, home and specialized settings. Works in close collaboration with staff, consumers and family in safety planning and implementing treatment plans, supporting and navigating the system of care.
Provide crisis oriented individual and family counseling and support to participating patients in the emergency department, and inpatient service Conducts or participates in the client/ patient intake process to obtain information required to formulate a psychosocial evaluation and social work treatment plan Provides timely and complete psycho/social assessments to all cases assigned and planning appropriate interventions. Conducts safety assessments and other standardized tools Interviews patients with a recent suicide attempt or high-risk suicidal behavior to assess he need for services; provides counseling and/or makes referrals as required Collaborate and assist emergency department and inpatient unit to develop the most appropriate discharge plan Demonstrate use of learned skills assessments and improvement in patient care Observe and identify verbal and non-verbal behaviors in clinical assessments. Identify and collaborate with existing support systems, facilitating community referrals as needed.
Facilitate groups and individual sessions with patients and their families Work with patients and families throughout the hospital and in the community, for up to 6 months. Participate in ongoing training and individual supervision Minimum Qualifications For Assignment to Level V 1. Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.
