The Lewis College of Nursing and Health Professions is seeking to hire a Sponsor Funded Professional - Social Worker II to join the team at Project Healthy Grandparents. As one of the eleven colleges at Georgia State University, the Lewis College encompasses various entities, including the School of Nursing, Perimeter College Nursing Program, and the Departments of Health Sciences, Nutrition, Occupational Therapy, Physical Therapy, and Respiratory Therapy.

WHAT WILL YOU DO:
As a Sponsored Funded Professional - Social Worker II, you will be an integral part of an interdisciplinary team focusing on direct assistance to grandparents raising grandchildren in parent-absent households. The primary service approach involves home-based case management, with a specific emphasis on families with grandchildren aged birth to five years who may be at risk for developmental delays.
DUTIES AND RESPONSIBILITIES INCLUDE:
- Conduct monthly case management home visits with families.
- Utilize the Ages & Stages Questionnaire to screen grandchildren aged birth to 5 years for developmental delays.
- Refer grandchildren with potential delays to the Neurodevelopmental Exposure Clinic at Emory University School of Medicine (ENEC) for a comprehensive evaluation.
- Assist grandparents in completing the application process for the developmental evaluation at Emory University.
- Accompany grandparents to ENEC for post-evaluation meetings.
- Provide case management services to grandparents based on clinicians' recommendations.
- Offer individual parenting education support services to families.
- Recommend/provide developmentally appropriate toys, books, or educational resources to grandchildren.
- Collaborate with grandparents and school personnel for school-age grandchildren's Individual Education Plans (IEP).
- Prepare monthly program service reports, progress notes, and maintain accurate documentation using computer database and/or spreadsheet programs.
- Coordinate scheduling for service referrals and transportation for families as needed.
- Interface with Project Healthy Grandparent’s interdisciplinary team and other community partners.
- Attend departmental meetings, trainings, and required workshops.
- Organize and facilitate monthly support group and parenting group meetings as needed.
SPONSOR FUNDED:
***This is a regular, full-time position which involves on-campus and online duties, with job continuation dependent upon grant funding.
Qualifications
Bachelor's degree and three years of related experience; or Master's degree and one year of related experience; or a combination of education and related experience.
