JOB SUMMARY
Supports patients and their family's physical and emotional needs and provides resources, and access to supplemental resources during end-of-life care. Reports to Hospice Director. Non-Exempt position.
Key Responsibilities:
- Psychosocial Assessments: Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and financial needs.
- Care Planning: Develop and implement individualized care plans in collaboration with the interdisciplinary team, addressing the unique psychosocial aspects of each patient's situation.
- Counseling: Provide counseling and emotional support to patients and families facing end-of-life issues, grief, and bereavement.
- Resource Coordination: Assist patients and families in accessing community resources, financial assistance programs, and support services.
- Advance Care Planning: Facilitate discussions on advance care directives, assisting patients in expressing and documenting their wishes for end-of-life care.
- Collaboration: Work collaboratively with physicians, nurses, spiritual counselors, and other healthcare professionals to ensure coordinated and holistic patient care.
- Bereavement Support: Offer bereavement support to families, providing counseling and connecting them with appropriate resources during the grieving process.
- Documentation: Maintain accurate and up-to-date documentation of assessments, care plans, and interventions in patient records.
- Education: Provide education to patients, families, and staff on social and emotional aspects of end-of-life care.
- Advocacy: Advocate for patients and families to ensure their psychosocial needs are met within the healthcare system.
Qualifications:
- Must be Licensed Master Social Worker (LMSW) or Licensed Baccalaureate Social Worker (LBSW) in the State of Texas.
- Master's Degree in Social Work, Bachelor's Degree in social work, or Bachelor's degree in psychology, sociology, or other field related to social work required.
- Valid Texas Driver's License, automobile insurance, and reliable automobile required.
- Must be able to communicate effectively in English, both verbally and in writing. Additional languages are preferred.
- Must possess high degree of organization and be able to multi-task efficiently and effectively.
- Computer knowledge required, including proficiency in Microsoft Office and Outlook.

Functional Job Requirements:
Job requirements include, but are not limited to: sitting, standing, walking, kneeling, bending, squatting and reaching. Requires good finger dexterity, repetitive motions with hands and fingers. Requires ability to have good vision and hearing. Pushing and pulling equipment may be necessary.

Job requires medium lifting on an occasional basis.
