Guidant Care Management is currently seeking qualified applicants for the position of Care Coordinator Social Worker. You must have a Master’s Degree in social work, psychology counseling, rehabilitation, gerontology, or sociology plus 1 year of related senior case management experience. The primary responsibility of the Care Coordinator Social Worker is to perform monthly client visits and assess the care needs for Medi-Cal beneficiaries enrolled in the California Assisted Living Waiver Program. You will be working with seniors and persons with disabilities that require nursing facility care, but can safely live in an Assisted Living Facility (RCFE).
The position requires both office and field work and traveling to Assisted Living Facilities throughout San Diego County. The Care Coordinator Social Worker maintains a comprehensive understanding of ethical principles and independence in their work, professional conduct, duty, and responsibilities. Competitive Per Diem with mileage reimbursement.
Essential Duties and Responsibilities:
- Perform monthly visits to Medi-Cal clients.
- Create Monthly Progress Notes.
- Create Action Items for Residential Care Facilities for the Elderly (RCFE.)
- Telephone and email RCFE’s on key issues.
- Identify community resources to assist Medi-Cal clients.
General Position Requirements:

- Ability to coordinate and organize multiple tasks.
- Ability to work independently.
- Have own transportation, auto insurance coverage & clean DMV driving record.

- CPR/First Aid Certification
