Job description

The Social Services Coordinator serves as a liaison for residents with internal and external health care providers for moves throughout the continuum, including visits to the hospital when appropriate. The Social Services Coordinator promotes a positive customer experience and coordinates with the appropriate interdisciplinary teams when a resident is in transition to the next level of care or discharge.
This position will be scheduled 36 hours per week Monday-Friday, eligible for full-time benefits, and paid hourly (non-exempt).
Essential Responsibilities and Duties
1. Develops an initial care plan based on the physical and psychological needs of the resident.
2. Continuously updates resident care plans and documentation regarding resident condition.
3. Works closely with the healthcare team to identify, assess, and address any needs that can be met either through Social Services, a chaplain, or a mental health provider.
4. Assists the resident in adjusting to the community and promotes continuity of relationships with family and community. Develops relationships with community resources to best serve residents and families.
5. Coordinates relocation planning, including advising and referring to community resources before and during relocation. Coordinates transfers of residents from the residential building to the health center.
6. Participates in QAPI, Quality of Life, and Medicare meetings and processes as assigned by the director of health services.
7. Knows the RAI process and performs MDS assessments as needed. Supports the timely and accurate completion of the MDS for all assigned residents.
8. Educates staff regarding the psychosocial and behavioral needs of residents.
9. Completes and maintains compliance with PASRR and OBRA.
10. Participates in surveys, advanced directive planning, and treatment preference options.
11. Utilizes clinical judgment and expertise to coordinate care management/behavioral health interventions and appropriate referrals.
12. Possesses understanding of technical and clinical criteria required to cover Medicare beneficiaries in Skilled Nursing.
Minimum Required Qualifications
Must be one of the following:

  • A person with a bachelor's or master's degree in social work.
  • A person with a bachelor's degree or master's in a healthcare or nursing related field.
  • A person with a bachelor's degree and a minimum of 2 years of experience working in a Long Term Care setting.
  • A person with a bachelor's or master's degree with a specialty in gerontology.
  • An ordained minister, priest, rabbi, or sister/brother of a religious institution.
