Job Description
General Responsibilities:
Provide case management services, financial resources counseling, crisis intervention, supportive counseling. Support and advocate for patients to reduce barriers to health care.
Required Qualifications:
- Excellent written and oral communication skills
- One year of social work case management experience with high-risk families and individuals
Physical Effort/Environment:
Moderate physical activity with mobility required to enable meeting with patients throughout the health center as well as occasional home visiting required. Writing ability required for patient records. Computer skills also required.
Work Schedule Demands:
As scheduled. Some evenings required. May require flexibility when necessary for patient crisis.
Communication Skills:
- Requires excellent oral and written communication skills at all levels of client arm staff interaction.
- Documents clearly and concisely all interactions in the medical and social service record
- Must be able to communicate as a part of the clinical team
Confidentiality of Information:
- Full access to clients' medical record and some access to financial information.
- Access to documentation from outside agencies.
- Maintain client confidentiality at all levels of interaction.
Significant Job Functions:
Clinical
- Functions as a part of the clinical team accepting referrals from medical providers and other agencies.
- Provides psychosocial assessment and determination the need for case management services.
- As a part of case management, provides necessary referrals for services such as mental health and substance abuse counseling, DCYF, HIV/AIDS case management, Health Care for the Homeless, Family Planning, and others as appropriate.
- Develop a treatment plan with the client, setting goals and objectives.
- Participate in clinical supervisions, presenting cases for review.
- Develop, implement, and coordinate psychoeducational programs targeting various public health prevention initiatives.
- Provides prenatal assessment and counseling and referrals to community agencies.
Organizational
- Participate in community committees and initiatives as appropriate.
- Participate on THC committees as needed.
