Position Overview
The Division of Developmental Disabilities is seeking to hire a Social Service Program Planner III for the Northern Illinois Region located in Chicago, Illinois to coordinate and monitor programs implementation for community providers. Provides consultation to agencies in resolving problems or improving services. Plans and coordinates the Social Service system studies by creating and/or editing Microsoft documents. Conducts programmatic and fiscal monitoring reviews of DHS-funded agency programs by conducting on-site visits to each agency in the assigned area as directed by the Northern Illinois Regional Facilitator. Travels in the performance of job duties.
Job Responsibilities
  1. Coordinates and monitors program implementation for community providers.
  2. Plans and coordinates the Social Service system studies by creating and/or editing Microsoft documents.
  3. Conducts programmatic and fiscal monitoring reviews of DHS-funded agency programs by conducting on-site visits to each agency in the assigned area as directed by the Northern Illinois Regional Facilitator.
  4. Processes requests for DHS-funded residential funding requests for DHS-funded community services.
  5. Responds to requests for technical assistance from individuals and service/support agencies within the assigned area.
  6. Evaluates and deflects possible admissions to state-operated facility services according to MHDD code criteria.
  7. Assists the Regional Facilitator with the coordination of Regional activities with the Network Management Team in order to facilitate professional, efficient operations within the Region.
  8. Responds to correspondence inquiries.
  9. Performs other duties as assigned or required which are reasonably within the scope of the duties enumerated above.

Minimum Qualifications
  1. Requires knowledge, skill and mental development equivalent to the completion of master's degree.
  2. Requires one (1) year of progressively responsible professional experience in community organization, Social Service work or in program planning and development.

Preferred Qualifications
  1. Two (2) years of professional experience coordinating and monitoring program implementation for community providers for a public or private organization.
  2. Two (2) years of professional experience receiving, approving and monitoring the implementation of development plans and conducting reviews of contract management.
  3. Two (2) years of professional experience conducting on-site visits and preparing summary reports.
  4. Two (2) years of professional experience analyzing and evaluating programs and operations.
  5. Two (2) years of professional experience providing technical assistance to individuals with developmental disabilities, their family members and service providers.
  6. Two (2) years of experience communicating effectively both verbally and in written form.

Conditions of Employment
  1. Requires the ability to meet criteria for a Qualified Intellectual Disability Professional (QIDP).
  2. Requires the ability to travel with overnight stays as appropriate.
