Description
ProMedica Senior Care, formerly HCR ManorCare, provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.
Responsible to provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Educational Requirements
Centers with more than 120 Beds: Bachelors in Social Work or a Bachelor's in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology; Licensed where required by law.
Centers with 120 Beds or less: High School Diploma required; Bachelors in Social Work or a Human Services field desirable; Licensed where required by law.
Position Requirements
For Centers with more than 120 Beds: One year of supervised Social Work experience in a health care setting working directly with geriatric individuals. For Centers with 120 Beds or less: Two years of supervised Social Work experience in a health care setting working directly with geriatric individuals desirable.
