Overview:

With the supervision of a Licensed Clinical Social Worker (CT) or a Licensed Independent Clinical Social Worker (RI), the social worker will perform professional social work services to patients, and families, and social work consultation to the interdisciplinary teams in the Yale New Haven Health System. Supervision means face-to-face case review and consultation between one supervisor and one supervisee until the supervisee obtains Licensed Clinical Social Worker in accordance with the State of Connecticut regulations/licensure for Licensed Independent Clinical Social Worker in accordance with the State of Rhode Island regulations.

Qualifications:

EDUCATION


Master's Degree from an accredited School of Social Work.


EXPERIENCE


Minimum of one field placement from an accredited School of Social Work.


LICENSURE


Licensed Master Social Worker (LMSW) from the State of Connecticut, Licensed Clinical Social Worker (LCSW) from the State of Rhode Island, or those holding licenses from other states, will need an 'endorsement' and meet the State of Connecticut or Rhode Island license requirements.


SPECIAL SKILLS


The Social Worker will possess the following special skills: empathy, active listening, critical thinking, strong organizational and computer skills, and the ability to effectively communicate with and advocate for patients and families.


PHYSICAL DEMAND


The Social Worker-LMSW/GF position is co-terminus with grant funding.


Additional Information:
LMSW required. Must be a 2024 Sacred Heart School of Social Work CT CARES program graduate.
Position is coterminous with funding 7/1/2024-6/30/2024
Work within psychiatric emergency and inpatient services with individuals, ages 16+, with mental health, substance use and co-occurring disorders.
Complete psychiatric evaluations, inclusive of mental status examinations, risk assessment and provisional diagnosis as well as psychosocial assessments.
Complete and implement treatment plans in collaboration with interdisciplinary treatment team.
Provide crisis intervention and stabilization; individual, family and group intervention; safety planning; clinical case management; and disposition planning.
Provide social work consultation to interdisciplinary treatment team.
Document in the Electronic Medical Record (EMR).
