Case Management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s needs. The Case Manager(s) will work to address the physical, psychological, and social needs of clients, and help them achieve self-sufficiency. This position will be focused on our adult shelter clients.
Primary responsibilities will include, but are not limited to:
- Provide personalized one-on-one support to clients
- Make referrals to community resources and assure linkage with those resources
- Assist clients in identifying and achieving their personal goals
- Monitor, evaluate, document, and track the progress of clients, and revise case plans as needed
- Offer educational and vocational services to clients
- Establish a system for finance management
- Support clients in their recovery from substance abuse and other forms of trauma
- Help manage client crises
- Assist with the building of community living and life skills programming
- Collaborate closely with external agencies, schools, and community partners to enhance support networks
- Commit to serve our clients with excellence in light of our mission and values.
Qualifications
Desired:
- High school diploma and 5+ years of Case Management experience , OR
- Bachelor’s Degree and 1+ years of Case Management experience

Job Type: Full-time
Pay: $21.00 - $25.00 per hour
Expected hours: 32 – 40 per week
