Requirements
MINIMUM QUALIFICATIONS
  • Associate degree or equivalent experience
  • At least 1 year experience providing case management with adults or families in one-on-one and group settings.
  • Familiarity and experience in client interviewing, assessment, and service plan development.
  • Familiarity and experience working in community-based agencies
  • Excellent organizational skills and demonstrated follow-through on tasks: multi project/multitask orientation.
  • Must be available to work on site at Islais Place 8:30 am to 5 pm or 10:30 am to 7 pm Monday-Friday and some weekends as needed.
  • Conflict resolution skills.
  • Proven ability to work with people from a variety of different ethnic, socioeconomic, educational, religious, sexual, and generational backgrounds.
  • Strong interpersonal, written and verbal communication skills.
  • Case Management skills, oral communication skillswith families and youth, and the ability to clearly document outcome and data per program protocols
  • Professional demeanor, pleasant personality, and a sense of humor
  • YMCA approved CPR and First Aid Certification

PREFERRED QUALIFICATIONS
  • Bachelor's degree or equivalent experience
  • English/Spanish or English/Cantonese fluency

Position Description
Urban Services YMCA's programs recognize and re-affirm client strengths, encourage meaningful and collaborative participation in program creation and delivery and recognize clients and communities as the best resources for overcoming challenges and identifying solutions.
Under the supervision of theAssociate Director of Transitional Services (ADTS), the Service Connector (SC) is responsible for supporting the ongoing development and implementation of the Service Connector Program (SCP) at Islais Place 4840 Mission St., Additionally,the SC conducts intake, provides appropriate information as part of SCP outreach, provides referrals to support family needs, and support with all aspects of relocation. The SC serves as a bridge between families and the property manager offering targeted family support programming. The SC assists Cobb Elementary families to access resources through City agencies, local business, cooperative and collaborative Community Based Organizations. The SC will create and provide workshops tailored towards the needs of the Islais Place Community. The SC works as a team member with the other YMCA staff to ensure a holistic approach to the family needs. Much time and energy are spent incase management approaches. The SC will collect, enter and report data into Service Connector databases. They must also follow the curriculum provided, manage incentives, follow up with families and outreach. Also required to participate in SCP core events.
This position is concerned with the delivery of services in the Islais community. The SCadheres to all confidentiality procedures. Service Connectors are mandated reporters and will complete training within 90 days of employment.
