Requirements
MINIMUM QUALIFICATIONS
- Associate degree or equivalent experience
- 1+ year of experience providing case management with adults or families in one-on-one and group settings.
- Familiarity and experience in client interviewing, assessment, and service plan development.
- Familiarity and experience working in community-based agencies
- Excellent organizational skills and demonstrated follow-through on tasks: multi project/multitask orientation.
- Must be available to work on site at Cobb Elementary 8:30 am to 5 pm or 10:30 am to 7 pm Monday-Friday and some weekends as needed.
- Conflict resolution skills.
- Proven ability to work with people from a variety of different ethnic, socioeconomic, educational, religious, sexual, and generational backgrounds.
- Strong written and verbal communication skills.
- Professional demeanor, pleasant personality, and a sense of humor.
- YMCA approved CPR & FA Certification
PREFERRED QUALIFICATIONS
- Bachelor’s degree or equivalent
- English/Spanish or English/Cantonese fluency

Position Description
Under the supervision of the Beacon Director, the Service Connector (SC) is responsible for supporting the ongoing development and implementation of the Service Connector Program (SCP) at Cobb Elementary. Additionally, the SC conducts intake, provides appropriate information as part of SCP outreach, and provides referrals to support family needs. The SC serves as a bridge between families and local service providers offering targeted family support programming. The SC assists Cobb Elementary families to access resources through City agencies, local business, cooperative and collaborative Community Based Organizations. The SC works as a team member with the other YMCA staff to ensure a holistic approach to the family needs. Much time and energy are spent in case management approaches. The SC will collect, enter and report data into Service Connector databases. They must also follow the curriculum provided, manage incentives, follow up with families and outreach. Also required to participate in SCP core events.
This position is concerned with the delivery of services in the Cobb community. To this end, the SC must have the interpersonal skills to interact effectively with families identified. Case Management skills, oral communication skills with families and youth, and the ability to clearly document outcome and data per program protocols are all necessary. The SC adheres to all confidentiality procedures.
