Summary
Incumbent serves within the Homeless Programs under Social Work Service (SWS) at the Richard L. Roudebush VA Medical Center in Indianapolis. The Senior Social Worker is the Contract Residential Services Liaison for Homeless Programs. Position enables VA Homeless Programs to fully collaborate with the community for contracted residential services programming, including two Continuums of Care (CoCs) and community partners from the 32 counties served under the VA Indianapolis catchment area.

Duties
Duties include but are not limited to:
Administrative Functions
  • Develops, implements, and maintains all established and new contract programs.
  • Oversees the establishment of program monitors and outcome measures within the assigned program(s) using a data-driven quality assurance process, in, monitoring outcome measures and metrics.
  • Is responsible for Contracting Officer Representative duties, maintaining at least a COR II designation, complies with conflict-of-interest laws, Executive Branch Standards of conduct to avoid conflicts of interest while serving as the CR liaison.
  • Complies with all commonly accepted fiscal practices that are subject to OIG and GAI review.
  • Advocates on behalf of Veterans' service in Homeless Programs before local, state, regional, and federal entities
  • Monitors the clinical care provided by contracted agencies.
  • Provides contracted agencies with necessary technical assistance, when and where appropriate.
  • Reads and enforces the contractor's approved contract and works closely with the
    contracting officer to ensure compliance.

Clinical Functions
  • Serves as the Lead liaison (Point of Contact) for the HCHV Programs team for the Indianapolis Medical Center on homeless Veterans in VA ER/hospitalized/inpatient needing Homeless Outreach Services for emergency housing access.
  • Provides clinical care, coordination and case management services by individual or group to Veterans, which includes referral, linkages to various treatment programs within VA and non-VA services.
  • Conducts an admission assessment, clinical reminders, and ongoing screening to inform the contract provider on treatment planning development and changes.
  • Provides crisis intervention.
  • Facilitates and reviews the development of treatment/care plans for Veterans with contract agencies.
  • Completes case notes in VHA health record.
  • Educates Veterans and/or their caregivers about VA and community resources to address their needs.
  • Completes diagnostic tools including the Homeless Operations Management and Evaluation System (HOMES) assessment and HMIS assessment to ensure appropriate housing resources and additional treatment services are received
  • Consults, collaborates, and coordinates with other staff who provide care to Veterans.
  • Reviews relevant data, assesses the data, develops ideas, and implements plans for improvement.
  • Participates in the training of medical students, medical and psychiatric residents, nursing students, MSW Trainees, fellows, and other graduate or post-graduate trainees, as appropriate.
  • The Social Worker may coordinate or assist with the coordination of the Healthcare for Homeless Veterans Dental Program.
  • Works collaboratively with Vocational Support services to Veterans in CR programs which may help Veterans to navigate vocation and education support agencies.

Qualifications
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education Requirement - You must have a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work.
  • Licensure - You must be licensed or certified by a state to independently practice social work at the advanced practice level, and must be able to provide supervision for licensure. Indiana requires Licensed Clinical Social Worker-LCSW.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Experience: Must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition to the experience above, the candidate must demonstrate the following knowledge, skills, and abilities:
  • Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
  • Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
  • Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
  • Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
  • Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

Physical Requirements: The physical demands of the position involve standard office requirements such as using the telephone, retrieving files, writing reports, data entry and maintenance of computer databases, etc. Work is typically performed in the community (community contract provider, shelters, other community settings). This position requires regular occasional travel. The incumbent must maintain a valid driver's license and abide by requirements for operating a Government Owned Vehicle. The position requires periods of walking, standing, sitting and bending.
