Your Specific Responsibilities:

This is a professional position that serves as the Patient Care Medication Adherence and Training Coordinator within the Patient Care Unit of the HIV/AIDS Section, Bureau of Communicable Diseases, Division of Disease Control and Health Protection. This position reports to the ADAP Operations and Compliance Manager. The position is vital in implementing activities to improve medication adherence throughout the state, providing programmatic and technical training to county ADAP staff, and ensuring compliance with federal law, policy, and state eligibility rules and requirements. This position collaborates with HIV/AIDS Section staff and county health departments to plan and coordinate activities to improve ADAP clients' health outcomes.

 

The Medication Adherence and Training Coordinator will work with ADAP regional coordinators and county DAP staffing analyzing program data to monitor client compliance, medication, and recertification adherence. This position will work with individual counties and the regional coordinators to identify challenges unique to the area to enhance program services and delivery methods and overcome adherence barriers. Confirms program policy adherence and collaboration efforts with local CHO and Florida Department of Health (FDOH) linkage program engagement specialists for client re-engagement. Works with CQM efforts to identify barriers and incorporate the strategies for improvement of access to services for clients, resulting in increased medication adherence. Assess and identify county health department needs in providing services to eligible ADAP clients, provide technical assistance, and determine any visits/training needed for the area. Monitor and document county health departments' compliance with program standards and make recommendations for technical assistance/quality assurance and improvement. Ensures that any information and technical assistance provided to the field is consistent and aligned with the program's policy and procedures.

 

Use QI tools and best practices to identify strategies and develop plans to increase client compliance in alignment with program standards. Gain an understanding of the programs available in the Prevention Unit and how to leverage those programs to enhance adherence strategies and engage ADAP clients. Documents and shares knowledge of best practices with areas of the state with documented lower rates of medication adherence performance and collaborates with county health department staff in these areas to develop performance improvement plans utilizing best practices, baseline data, setting targets for improvement, and timeframes for performance measurement. Routinely follows up with county health departments to ascertain progress with the performance improvement plan, key successes/challenges and provide technical assistance. Assists ADAP state health office team members to review monitoring results to ensure compliance with federal law, policy, and state eligibility rules and requirements.

 

Responsible for the introductory, intermediate, advanced-level, and ad-hoc programmatic, eligibility, and technical training for Patient Care Programs. Draft training materials, which may include print, media, and video tutorials, and explore avenues for effective delivery of program education and instruction to enhance program knowledge, skills, and competencies of CHO ADAP staff, ADAP clients and stakeholders. Coordinates training components with the HIV/AIDS Section. HIV/AIDS Patient Care, Communications and Health Equity Unit and the Department's Office of Communications. Coordinates with the HIV Prevention Program to provide technical assistance and training to Minority AIDS Initiative (MAI) -funded providers and identify opportunities for programmatic collaboration.

 

Participates in developing and implementing planning, policies, procedures, rules and standards required to manage DAP and enhance program knowledge skills effectively and competencies.Provides feedback on the ADAP Program Policy and Procedures manual to ensure grant compliance and consistency with HRSA's National Monitoring Standards for Ryan White Part 8 Grantees, including but not limited to medication formulary management, eligibility determination and client enrollment and access to services. 

 

Assists in drafting and communicating program updates and policy clarifications. 

 

Responsible for the development and accurate updates of the program's central office procedures and process maps.

 

Travel as assigned to complete tasks stated within this position description.

 

Performs other related duties as assigned.

Qualifications:

Preferred - associate degree.

3-5 years of experience
