- Has advanced Heart Failure
- Has developed some cognitive impairments (delirium). Sometimes she can't tell you what day it is.
- Doing speech therapy to help with cognition.
- Needs assistance with all ADLs
- Assistance with eating and meal preps
- Can scoot out of bed with assistance .Can stand and walk a few steps with a walker. Everyday she is gaining strength.
- Can pivot, but it's not easy for her to walk.
