Summary
The SDOH Coordinator position combines the work of addressing patients identified Social Determinates of Health needs and providing short term/low acuity case management services to at risk populations of patients.
Working on the Complex Care Management team, this position will partner to connect patients to the specific SDOH needs they identified on the Partners in Care Questionnaires (PICQ) The team member shall possess experience at working with vulnerable adults and/or children and knowledge/skills in accessing programs and resources to address social determinants of health both within and outside of the Michigan Medicine. This position will also manage a case load of patients with ongoing needs.

Responsibilities*
  • Receives and Reviews Partners in Care Questionnaires (PICQ) for specified departments within Michigan Medicine Health system.
  • Provides referrals based upon identified patient needs to specific internal providers and/or external community agencies.
  • Coordinates care for high volume patient caseload with low acuity need
  • Provides direct coordination of care to patients though portal messaging and direct patient contact.
  • Follows up on identified needs and resources provided.
  • Provide basic transition of care services to selected patient populations.
  • Carries a small caseload of lower acuity patients requiring coordination of care
  • Assists with program operations by supporting Complex Care team members
  • Maintains an organized virtual work queue
  • Utilizes critical thinking skills and problem solves patients identified social needs
  • Builds relationships with community partners to help in coordination of care
  • Skilled with patient engagement
  • Provides transition of care services to select patient population; Ensures follow-up care is understood by patient and identified resources/follow up needs are in place (i.e., equipment, home health, other services, appointments, transportation, etc).
  • Provide a range of case management services to select patient population. Services provided will address barriers to care:
  • Provides relationship-based care to enhance trust and engagement in care
  • Coordinates care with Community partners (ie: Community Mental Health, Substance use providers, Housing/Shelter providers, Food pantries, etc)
  • Coordinates with internal Care Connect partners for resource need
  • Assist in obtaining resources for patients with identified barriers
  • Identify patients requiring higher level of care and coordinates referral to additional Care Connect providers (Swk, Pharm D, Nutrition, CCMP)
  • Participate/present in case conferences, clinical in-services, and other opportunities to enhance practice and strengthen care coordination services for patients.
  • Review cases with Medical Director and/or Program Manager on a regular basis to assess goals and review case closure criteria.
  • Carries caseload of lower-level acuity patients that require constant and time limited care coordination.
  • Maintains all required documentation in the appropriate electronic systems in a timely basis.

Required Qualifications*
  • Bachelor?s degree (BSW), Associates degree in social sciences or other related field, or an equivalent combination of education and experience.
  • Skilled at addressing Social Determinant of Health needs
  • Familiar with community resources and connecting individuals to a variety of agencies/resources
  • Knowledge of medical terminology including experience in a health care environment.
  • Well-developed interpersonal and communication skills, including excellent verbal and written communication skills.
  • Ability to perform in a self-directed manner within a team-focused, participative management framework.
  • Excellent time management and organizational skills to prioritize workflow.
  • Strong analytical skills to evaluate social service needs of patients
  • Experience with computer applications and equipment

Background Screening
