SalaryFull-time position for a School Based Case Manager at J.P. King, Jr. Middle School in Franklin. Will assess student needs, provide individual and family counseling, academic support, behavior management, crisis intervention, home visits, transportation to appointments as needed and linkage to community resources for at-risk youth and their families.
Collaborate with J.P. King, Jr. staff, Franklin Public Schools, and community resources. Prepare reports for funding sources. Develop and implement summer enrichment program. Majority of work hours during the school day but also afternoon and some evening hours in the community. Occasional weekend hours if necessary. The Commission compensates counselors for travel, administrative, and case management time.
