AuthoraCare Collective is currently seeking an RN/LPN for our Palliative Care/Chronic Disease Management position. This is a full-time position (8:00am - 5:00pm) supporting our Greensboro campus.

Job Summary:
Provide high quality clinical support to the palliative care and Chronic Disease Management of and direct care of patients through face-to-face visits, virtual visits or telephonic assessments, verify continued care is necessary, scheduling patients to be seen, and following up with patients as requested. Address the physical, psychological, emotional and spiritual needs of individuals with life-limiting or symptomatic issues.
Assess and identify patient/caregiver issues, help guide and develop patient/caregiver care goals, and identify and implement interventions as appropriate for given setting. Duties will also include coordination of care with internal/external healthcare resources, education and assisting with program development. Communicate effectively both verbally and in writing with the AuthoraCare (ACC) Nurse Practitioners, Chief Medical Officer, and community providers. Assess and identify patient needs primarily in the home. Document the outcome of care in the appropriate EMR system.
