Job Description:

Optimum VIP Care Services is looking for Registered Nurse (RNs) and Licensed Practical Nurses (LPNs) to join our team. Requirements:

● Active RN/LPN LICENSE IN THE STATE OF GEORGIA (Required)

● Current CPR certificate

● First Aid training

● Valid driver’s license

● TB and Hepatitis Acknowledgement

● Proven working experience as a Home Health Aide

● Access to work independently and with directions

Essential Tasks and Responsibilities

● Makes care rounds and reports pertinent information as appropriate. Takes vitals and tube feeding.

● Provides personal care such as bathing, shampoo, nail care, mouth care, as directed by the patient's plan of care in accordance with service procedures and assists patients with necessary self help skills, leaving the patient's immediate environment neat and clean.

● Utilizes proper body mechanics, provides for positioning, mobility, or comfort needs. Contributes to patient safety and reports potential problems/concepts to the Patient Care Manager/RN/Therapist.

● Ensures resident activity and other worksheets are completed appropriately.

● Stores equipment and supplies in compliance with policies and procedures.

● Reports all complaints and grievances made by patient or family members to charge nurses.

● Responsible for storing resident’s personal items properly.

● Assist in washing residents clothes.

● May assist with housekeeping duties.

Responsibilities

Assisting clients’ condition during every visit and track any observation.

Take and record measurements of vital signs such as blood pressure, temperature, heartrate etc.

Administer medication as prescribed by the client’s physician.

Dress or undress open wounds and assess progress of healing.

Continuously educate clients, their families and other caretakers on proper Home Healthcare strategies and procedures.

Provide palliative care as needed to keep the client comfortable.

Make recommendations for devices or tools that might improve the client’s quality of life. Listen to clients and be prepared to respond to concerns or requests.
