Responsibilities
The Total Joint Nurse Navigator (TJNN) is an ambassador, educator, and experience guide for the surgical patient as well as family members. The TJNN is responsible for facilitating the patient experience by guiding patients across the care continuum. The TJNN is the primary point of contact with the patient and serves as the liaison between the patient, provider, and facility. The TJNN provides leadership for all aspects of the continuum of care beginning before surgery through the post-operative phase, including Pre-Admission Testing, Peri-op Services, inpatient units, post-acute care. The TJNN will assist the program coordinator in the planning, development, and implementation of program goals and operations.
This position will work collaboratively with Treasure Valley Hospital physicians, the Total Joint Program Coordinator, patients, families, and staff to develop and maintain a multidisciplinary Joint Replacement Program.
Responsibilities
  • Manages the transition of total hip and knee arthroplasty patients across the continuum of care.
  • Ensure appropriate pre-op preparation for every total hip and knee arthroplasty patient.
  • Manages patient demographic information in Recovery Coach
  • Utilizes Recovery Coach for patient education, communication, and data collection.
  • Coordinates and manages the Total Joint Class content, schedule, and instructors.
  • Facilitates communication between patients, facility, and surgeons.
  • Ensures patients and their families are provided pre- and post-op education and educational resources.
  • Supports the patient and family in coping with the psychological aspects of their surgery
  • Eliminates barriers for patients to optimize care.
  • Collaborates with other service line members.
  • Serves as the primary point of contact between the patient, their family members, and the care team.
  • Organizes, reviews, and provides feedback for program protocols, care plans, and patient educational offerings.
  • Assists in the collection and analysis of performance metrics data.
  • Serve as a clinical resource for facility and surgeon offices
  • Adheres to TVH's standards, protocols, and policies.
  • Maintains position's required education and competencies.

Qualifications
  • State Professional Registered Nurse License
  • Nurse Navigator Certification
  • BLS
  • Bachelors degree in Nursing, required
  • Total Joint Replacement experience preferred
  • RN 2. 5 years nursing experience in orthopedics.
USD $22.45/Hr. USD $39.95/Hr.
