Job Description: RN Supervisor for Home Health
Position Overview: We are seeking an experienced and compassionate Registered Nurse (RN) Supervisor to oversee and coordinate our Home Health nursing team. The RN Supervisor will provide clinical leadership, guidance, and support to ensure the delivery of high-quality, patient-centered care in the Home Health setting. Responsibilities:
- Provide clinical supervision and oversight to a team of Home Health nurses, including RNs, LPNs, and CNAs, ensuring compliance with agency policies, procedures, and regulatory standards.
- Conduct initial and ongoing assessments of patient needs, collaborating with interdisciplinary team members to develop individualized care plans that promote optimal health and independence.
- Coordinate patient care activities, including scheduling visits, assigning caseloads, and ensuring timely and efficient delivery of services in accordance with physician orders and care plans.
- Monitor and evaluate patient outcomes, clinical progress, and adherence to treatment plans, intervening as needed to address changes in condition or care needs.
- Provide clinical expertise and guidance to nursing staff regarding complex patient cases, medication management, wound care, and other clinical interventions.
- Conduct regular supervisory visits to patient homes, assessing the quality of care provided, addressing any concerns or issues, and providing education and support to nursing staff as needed.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals involved in patient care to coordinate services, facilitate communication, and promote interdisciplinary collaboration.
- Ensure accurate and timely documentation of patient assessments, interventions, and outcomes in electronic health records systems, maintaining confidentiality and adhering to HIPAA regulations.
- Participate in agency quality improvement initiatives, performance improvement activities, and clinical audits to monitor and enhance the quality of care provided by the Home Health team.
- Serve as a resource and mentor to nursing staff, providing ongoing education, training, and support to promote professional development and competency in Home Health nursing practice.
Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of two years of clinical experience in Home Health, community health, or a related healthcare setting.
- Previous supervisory or leadership experience preferred.
- Strong clinical assessment and critical thinking skills, with the ability to make sound clinical judgments and decisions.
- Excellent communication and interpersonal skills, with the ability to effectively interact with patients, families, caregivers, and interdisciplinary team members.
- Proficiency in electronic health records systems and computer skills.
- Knowledge of Home Health regulations, reimbursement guidelines, and accreditation standards.
- Commitment to providing compassionate, patient-centered care and promoting the highest standards of nursing practice in the Home Health setting.
