  • Non-exempt/Hourly
  • New hires are placed on our starting wage scale of $35.63- $43.65 per hour
  • Based on performance, there is room to grow on our full wage scale of $35.63 - $51.63 per hour

Sign-on Bonus:

  • $1,000 (subject to 1-year payback clause)

Work Schedule:

  • Schedules are set between 7:30am-6:00pm Monday-Friday with participation in a Saturday rotation working a Saturday every 4-6 weeks

Job Summary:

As a Pod RN, you would provide nursing care in an outpatient community health center as part of a multidisciplinary team. Primary responsibilities & duties include:


1. Ensures patient and staff safety and works to achieve and meet Continuous Quality Improvement (CQI):

a. Ensures safe work environment and promotes accident prevention.

b. Utilizes cause for concern form to identify situations that have an impact on care delivery, safety, or customer service.

c. Promotes care team achievement of quality measures and departmental goals.

d. Participates in development and delivery of team goals and objectives.


2. Performs direct patient care:

a. Performs venipuncture as necessary and in-house lab tests per standing order(s) as approved by the Medical Director.

b. Performs independent RN visits for both acute and chronic conditions in collaboration with provider.

c. Provides nursing care including patient education, assessments, nursing intervention, and medical case management.

d. In coordination with multidisciplinary team, develops and implements care plans for patients with complex medical and social problems.

e. Triages Same Day/Emergent needs (either by phone or “walk-in”) and refers patient to appropriate level of care.

f. Coordinates in collaboration with Care Team urgent or medically complex referrals.

g. Consults with outside providers, including nursing homes, pharmacies, and specialty providers.

h. Reviews urgent/critical lab results in coordination with providers to determine plan of care.

i. Communicates significant findings/changes in patient condition to the provider in a timely manner.

j. Documents patient care in the Electronic Medical Record (EMR) according to established standards.


3. Performs Care Management:

a. Identifies patients with high utilization, e.g., emergency room use, and provides resource allocation/case planning and coordination of services.

b. Assesses patient’s readiness to change, educational level, preferences and agreed-upon self-management goals.

c. Provides patient education/resources such as nutrition, lifestyle changes, sexually transmitted diseases, health care maintenance, chronic disease management and medication adherence and administration.

d. Completes Health Department reporting within required timeframe and required documentation.

e. Works with other POD RNs to cover absences and workload.

Requirements:

1. Current Washington State RN License required.

2. Current Basic Life Support (BLS) Certification required (if not currently certified, you can get certified upon starting).

Preferred Qualifications

  • Proficient with electronic clinical systems, including Electronic Medical Records and scheduling systems
  • Able to use Microsoft Outlook for basic email correspondence and calendar
  • Effective & respectful communication skills
  • Ability to understand and respond effectively and with sensitivity to all patients
