Job description

Overview:
The Outreach Nurse Coordinator works collaboratively with corporations, municipalities, organizations, and schools to coordinate onsite services by promoting health and well-being, through required health screenings and medical services in Cedar Rapids and surrounding areas. The coordinator will deliver onsite services with clinical care staff, physicians, and support staff. Onsite services will be performed in a timely and accurate manner with attention to detail from start to finish and with necessary follow-up. The coordinator will work to create a safe, healthy, happy, and stable work environment for UPH team members, business partners and their employees. The coordinator will do this as leader of the occupational health outreach team focused on the health needs of the customer to promote safety, reduce illness and injuries to team members; leading projects and programs that are beneficial to the needs of the business/industry through collaboration and a solutions-oriented approach.

Responsibilities:

Program Management – Employer Partnerships
  • Assumes primary responsibility for the plan of care for the patient or group of patients, per nursing standards of care.
  • Effectively delegates tasks and responsibilities to other health care team members and assumes responsibility for follow-up and collaboration.
  • Creates, implements, and organizes required health screenings to improve the health and safety of Employer team members.
  • Coordinates and recruits outreach health accounts for Cedar Rapids and surrounding areas.
  • Educates and counsels management and team members on employee health and safety related issues including trends, policies, procedures, and practices.
  • Continuously improves outreach and required health program screenings.
  • Maintains awareness of applicable laws and/or regulations affecting employee health and safety; evaluates compliance on an ongoing basis; remains current on law/regulation changes, recognizes implications to the organization and recommends course of action to ensure compliance.
  • Coordinates employee statistics and analyzes trends; coordinates distribution of data to stakeholders on monthly, quarterly, or annual basis; makes recommendations so that appropriate decisions can be made in collaboration with key decision makers.
Program Management – Onsite
  • Oversees all onsite programming for outreach purposes.
  • Works with Work Well leadership and company representatives to negotiate pricing and/or contracts.
  • Works with outreach team and company safety team in a multidisciplinary team approach (HR and Risk Mgmt.), maintaining compliance with administration and documentation of applicable employee required screenings, tests, and immunizations, in accordance with hospital policy and governing agencies.
  • Coordinates flu and vaccination clinics and ordering of all supplies needed and logistics.
  • Coordinates testing, treatment and needed follow up, for all required health screening in accordance with applicable regulatory agency for team members, outreach account employees and team members.
  • Responsible for the appropriate exchange of health information between primary stakeholders.
  • Keeps and maintains all records in accordance with OSHA regulations and guidelines.
  • Gathers statistics and analyzes trends; makes recommendations so that appropriate decisions can be made in collaboration with providers and company representatives (HR, Safety, Risk Mgmt., etc.).

Patient Care:
  • Performs clinical tasks with clients and employees onsite and/or in the Work Well clinic:
  • Immunizations
  • Skin prick test
  • Vision screening
  • EKGs
  • Vitals
  • Hearing
  • UA collections
  • Coordination of care
  • OSHA reviews
  • Respiratory reviews/recommendations
  • Lab draws
  • Wellness screenings
  • BMI (height, weight, abdomen measurements)
  • Spirometry
  • Record review
  • Fit testing
  • Flu clinics
Qualifications:
  • Graduate of accredited nursing program.
  • 2 years of demonstrated progressive leadership experience.
  • Possess current RN licensure in the state of Iowa or compact state.
  • CPR certified
  • rites, reads, comprehends, and speaks fluent English.
  • Multicultural sensitivity.
  • Intermediate computer knowledge using EMR, word processing, spreadsheet, email, and web browser.
  • Use of usual and customary equipment used to perform essential functions of the position.
