The OB Patient Navigator program is an educational program for the OB patients that plan on delivering at our facility and to engage with the patients prior to 20 (at around 12-16) weeks through a list of OB clients provided by the OB providers, and to let them know we have a program available and that we offer childbirth, newborn, and breastfeeding education classes.
The OB Patient Navigator (Registered Nurse) will provide care to and in support of patients to include coordinating resources, scheduling appointments, coordinating referrals, follow up care, patient education, and other resources as identified to improve patient flow, patient compliance, patient satisfaction and referring provider satisfaction. The navigator will work closely with clinical staff/physicians, labor and delivery and clinic leadership to guide patients quickly to the appropriate area of care. This integral position is a part of the Family Care Services Department and is designed to decrease fragmentation of care and coordinate services. In addition to supporting the clinical and educational needs of the patient, the position will enhance patient access, quality of care, safety and ease of clinical evaluation for referring providers and services.
Dedicated to serving as the personal resource to the patient acting as a liaison to enhance the quality and continuity of care. Coordinates and guides patients and families throughout the continuum of their per-pregnancy and post-partum experience. Functions as an advocate, educator and care coordinator. Has the experience to assist patients and families with particular concerns and health care needs. Promotes and models superior customer service through effective and responsive communication methods. Assists with birth plans and lactation consulting.
Responsibilities:
  • Build a strong working relationship with referring clinicians.
  • Communicate with referring practices regularly.
  • Act as primary point of contact for referring offices needing emergent assistance.
  • Act as a liaison between the OB Physicians and ENMMC..
  • Maintain a list of patients who need navigation and perform the necessary coordination and care to improve their outcomes.
  • Provide tours of the unit
  • Be a point of contact for the patients
  • Coordinate additional care for the above patients to include follow-up testing, procedures, treatment, scheduling, physician's orders, and hospital admissions.
  • Provide individual nursing care and education to address the needs of the patient.
  • Maintain clinical competency in areas of responsibility to ensure quality patient care.
  • Assist in addressing patient questions and concerns.
  • Improve patient knowledge and understanding of treatment options and plans.
  • Promote informed patient decision making.
  • Ensure timeliness in scheduling patient appointments.
  • Manage patients through the electronic health record ensuring that patients are monitored through their active treatment plan.
  • Assist with tracking metrics and statistics as requested.
  • Document all patient encounters within the electronic health record.
  • Provide education and act as a resource to other staff members regarding the care of patients.
  • Assist in creating and teaching Prenatal classes
  • Participate in discussion and provide input regarding patient experience.
  • Assist with Chart Audits
  • Provide general and L&D/OB specific education to nursing staff
  • Assist Director with taking call
  • Attendance, as assigned, is essential to meeting required performance duties.
  • Complete other duties as assigned.

Requirements:
  • Associates Degree in Nursing, Bachelor's Degree in Nursing Preferred,
  • NM RN License, or Multi-State Compact allowing practice in NM.
  • BLS,
  • ACLS,
  • PALS,
  • NRP, and
  • STABLE.

CLC to be obtained within 6 months of date of hire.
