DEPARTMENT SUMMARY: The St. Charles Nurse Navigation and Care Transition team focuses on patient-centered transitions, which provides a seamless delivery of quality care from inpatient to the outpatient care team. The Ambulatory Care Nurse Navigation team collaborates with a multidisciplinary group to respond to the patient’s needs during transitions of care.
POSITION OVERVIEW: The Nurse Navigation Manager oversees the coordinated efforts to improve patient outcomes through the assessment and advocacy of patient needs. The Nurse Navigation Manager guides the team to provide evidence-based disease specific education, develop care plan goals and interventions, monitor and respond to barriers, and connect the patient to needed resources.
The Nurse Navigation Manager directly manages assigned clinical caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Oversees the assigned department to ensure smooth and efficient operations including allocation of resources.
Responsible for budget development, regular monitoring, accountability and meeting operational targets for all areas within span of control. Develops and implements annual plan including goals for unit with director.
Responsible for resource management ensuring appropriate and sufficient equipment and supplies are available. Manages staffing and scheduling and provides productivity oversight and review. Works with leadership to achieve productivity standards.
Hires, directs, coaches, and monitors the performance of all direct reports, to develop and maintain a high-performance team that meets organizational and department goals.
Monitors and ensures all direct reports are current with compliance and safety requirements. Implements and manages all organizational safety directives and goals.
Provides and oversees the team’s delivery of customer service in a manner that promotes goodwill, is timely, efficient, and accurate.
Serves as a real-time resource and assists with clinical expertise for caregivers problem-solving on patient care, processes, and family issues. Serves as representative for all customers.
Assists with quality, safety, customer service and regulatory compliance oversight: SAS (Safety Alert System) – Receipt, assignment, tracking and trending, delegation.
Coordinates quality initiatives and process improvements. Aligns and manages to St. Charles Health System metrics and quality initiatives.
Maintains ongoing communication with caregivers, department leadership, and others to ensure needs are being met for staff and patients. Engages medical staff to obtain input on the operation of the department and key decisions.
Supports the Director of Patient Flow in developing and maintaining physician and peer partnerships across the health system related to the achievement of campus and system strategic goals and patient flow metrics.
Communicates (both written and orally) effectively and maintains professional relationships with caregivers, medical staff, patients, guests, and other hospital departments.
Role models two-way communication of the St. Charles Health Care mission, vision, values, and strategic plan for the department (staff meetings, rounds, etc.)
Facilitates department meetings.
Oversees caregivers’ documentation to ensure proficiency in compliance with hospital policies and regulatory agencies.
Creates a healing environment that supports all aspects of care, respects the wholeness of the individual, and demonstrates therapeutic presence through attitudes and behaviors that enhance the care experience for patients and caregivers.

May perform additional duties of similar complexity within the organization, as required or assigned, including additional certifications.
EDUCATION:
Required: BSN required in related field.
Preferred: Master’s degree in related field.
LICENSURE/CERTIFICATION/REGISTRATION:
Required: Current Oregon RN license with the Oregon State Board of Nursing, BLS/CPR. Ability to travel to business functions/trainings/meetings, patient homes, and all St. Charles Health System worksites.

EXPERIENCE:
Required: Minimum five (5) years related experience; one (1) year leadership experience. Experience in a community health setting with knowledge of caring for chronic conditions.
Preferred: Experience in Care Coordination or Case Management.
PERSONAL PROTECTIVE EQUIPMENT:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
ADDITIONAL POSITION INFORMATION:
Excellent organizational skills, written and oral communication and customer service skills, particularly in dealing with stressful personal interactions.
Strong analytical, problem solving and decision-making skills. Ability to make independent nursing judgments, problem solves and prioritizes workload.
Basic to intermediate ability and experience in computer applications, specifically electronic medical records system, and MS Office. Demonstrates ability to maintain accurate notes and records.
Demonstrates evidence of essential leadership, communication, education, and collaboration.
Engages in an environment of continuous learning, embracing change and mastering competencies of new skills.
Coordinates external resources, assists patients in navigating through the healthcare continuum and works with disparate populations.
Maintains knowledge of chronic disease management and health promotion, case management, health prevention, government regulations, team process, quality improvement, and computer process.
Identifies and implements appropriate patient communication strategies and overcome accessibility barriers.
PHYSICAL REQUIREMENTS:
