We are looking for a PRN RN Case Manager to join our team! You will be responsible for the assessment, diagnosis, and implementation of health care plans for our patients. We are looking for RN's to cover the Metro Atlanta Area.
Responsibilities:
- Excellent written/verbal communication, problem solving and decision-making abilities
- Provide effective and efficient medical care while managing costs
- Diagnose and establish patient treatment plans
- Monitor and report changes in patient symptoms or behavior
- Promote assessment, intervention and teaching to maximize patient comfort
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Graduate from an accredited school of nursing
- 2 yrs. experience as a licensed Registered Nurse (RN) in the state and in good standing with the Board
- Familiarity with medical software and equipment
- A minimum of 2 YEARS HOSPICE experience
- Ability to thrive in a fast-paced environment
- Open to Full Time and PRN
