RN Hospice Admissions Nurse

Enhabit Home Health & Hospice is searching for a Hospice Admissions Nurse (RN) to join our team. This role will support our Hospice Admissions for our branches in Murray / Salt Lake & Ogden.

Hours: Monday-Friday, 10am-7pm

• Serve as the first point of contact and conduct initial meetings with patients and families to inform them about Enhabit hospice services
• Evaluate patients for hospice appropriateness, disceRN Hospice eligibility, and complete an initial assessment for those patients who meet criteria
• Educate and address any questions the patient and/or patient’s family has concerning hospice care
• Perform hospice admission visits
• Build effective referral source relationships to support the high quality and responsiveness expectations

• Must be a licensed Registered Nurse / RN in the state of employment.
• Must have at least one year of experience in hospice as an RN.
• Must have previous experience with Hospice Admissions, hospice eligibility, and hospice diagnosis specific criteria
• Must possess a valid state driver license, maintain automobile liability insurance as required by law, maintain dependable transportation, and must be able to safely drive in all types of weather.
• Must possess CPR certification for the healthcare provider.
