Job Description
- Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
- Prepare accurate, orderly, and timely documentation.
- Coordinate care with the patient and provide proper reports to the Case Manager.
- Provide health care instructions to the patient/family as appropriate per assessment and plan.

About You:
- You should have an active RN license in the state in which you are practicing, or be a new grad with eligibility to take NCLEX.
- Strong communication and interpersonal skills.
- Compassionate and empathetic approach to patient care.
- Ability to work independently and as part of a collaborative team.
- Reliable transportation and a valid driver's license.
