Rn Home Health Nurse

Supportive Home Health Care is a nurse owned skilled Home Health agency based out of Livonia, Michigan. Supportive Home Health Care is a Medicare certified Home Health agency. We are seeking a Registered Nurse who is a quick learner and is a team player.

** Previous use of WELLSKY (aka KINNSER) in the homecare setting is REQUIRED **

We are also looking for a RN who may want to start as contingent and work their way into a full-time or part-time caseload. Owner is also a Registered Nurses (RN) and is present daily.

Compensation

• $85.00 - $100.00 per visit
• OASIS - $110.00
• Weekends: $95 - $110 per visit (if needed based on patient frequency of visits)
• Orientation Period and In-Services: $30.00 per hour
• $5,000 sign on / retention bonus to be paid throughout the first year.

Job Description

• Manages a patient and family caseload which includes assessing, planning, implementation, evaluating and documenting the care provided.
• Plans, coordinates and delivers nursing care to patients and families.
• Supervises and documents Home Health aide/licensed practical nurse plans of care and documents supervisory visits.
• Participates in on-call coverage.
• Submits paperwork in a timely manner and updates and maintains patient medical records.
• Initial and ongoing comprehensive assessment of patient’s needs including Outcome and Assessment Set (OASIS) at appropriate time sets.
• Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed, or upon changes in the patient’s condition.
• Teaches, supervises and counsels the patient and family regarding home nursing procedures and other care needs as appropriate to the patient’s condition.
• Exhibits critical thinking and applies them for continuous improvement of services and the agency

Requirements:

• Active Michigan Registered Nurse (RN) license
• Valid driver's license and auto insurance
• A minimum of 2 years home care experience (preferred). Previous hospital/nursing home experience (will assess at interview)
• Familiarity with the Wellsky / Kinnser software REQUIRED (with minimal training as needed - will discuss)

Qualifications

• Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention.
• Ability to work well with an interdisciplinary team.
• Excellent written and verbal communication skills.
• Demonstrate problem solving and decision making abilities.
• Ability to develop and maintain rapport with the patient and family.

Travel

• Necessary on a daily basis; 100%.
