Qualifications:

• Valid and unrestricted Registered Nurse (RN) license in the state of California
• Minimum 1 year of nursing experience, preferably in Home Health, community health, or a related field
• Strong clinical assessment and critical thinking skills to make informed decisions and provide effective nursing interventions
• Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals
• Ability to work independently and manage time effectively to prioritize tasks and meet deadlines

Responsibilities:

• As a Home Health RN, you will play a vital role in providing high-quality healthcare services to patients in the comfort of their own homes and may involve wound care, medication management, medication administration, hands-on care to patients, intravenous therapy lines, observing and monitoring patients' conditions, maintaining accurate records and making physician recommendations.
• Maintain accurate patient medical records - you will be providing SOC Start of Care, Evaluation, Discharge / Re-certification
• Conduct comprehensive assessments of patients' health status
• Administer skilled nursing care to patients in their homes, including medication administration, wound care, monitoring vital signs, infusions, IV, and injections
• Coordinate and communicate effectively with patients, their families, and other healthcare professionals to ensure continuity of care and optimal patient outcomes
• Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote self-care and independence
