Job Description:
Our client is actively seeking a dedicated and professional Registered Nurse (Home Health) to be an integral part of their team. The chosen candidate will be instrumental in delivering exceptional Home Health care to patients within the Deerfield area. Your dedication, knowledge, and compassion will play a pivotal role in ensuring patients receive superior care in their personal spaces.
Primary Duties and Responsibilities:
- Offer comprehensive nursing care to Home Health patients in their residences.
- Liaise with a multidisciplinary team to devise an effective care plan.
- Continually assess, monitor, and document patient's health status and needs.
- Enlighten patients and their families on Home Health care procedures.
- Ensure accurate and timely electronic documentation of patient care.
- Engage in the company's in-depth 6-week RN onboarding program.
Required Skills, Qualifications, and Experience:
- Minimum of 1 year of professional experience as an RN.
- Active RN License in the State of IL.
- Associate's Degree (or higher) in Nursing.
- Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance.
Preferred Skills, Qualifications, and Experience:
- Home Health care experience.
Required Licenses, Training, and Certifications:
- Valid driver's license, reliable mode of transportation, and auto insurance.
- Valid Registered Nurse (RN) license/certification in Illinois (IL).
- Valid American Heart Association Basic Life Support (AHA BLS).
- Valid American Heart Association Advanced Cardiac Life Support (AHA ACLS).
(Note: BLS and CPR Certification assistance will be provided if not currently held.)
