Hybrid RN Nurse Evaluator
HealthCare Support is actively seeking a Hybrid RN Nurse Evaluator to fill an opening with a prestigious Health Plan in New York, NY.
Daily Responsibilities for Hybrid RN Nurse Evaluator:

• Develop Home and Community Based Services (HCBS) Plan of Care (POC) in collaboration with child and family that includes evaluation of health status.
• Perform evaluations of children that includes evaluation of health status, strengths, functional care needs, and preferences and guide the development of individualized long-term care service plans.
• Retrieve and review child’s medical documentation and or health referral forms, as relevant to the case.
• Utilize the State CANS assessment tool and MAXIMUS case management systems to document the child’s health status, strengths, functional care needs, diagnoses and preferences and transmit, as required.
• Shift: Monday – Friday, 8am to 5pm EST
  

• Incumbent New York RN licensure
• At least 1+ years of RN experience

• EMR experience
• Previous home health experience

• Immediate enrollment in Health Insurance

Pay Details: $48-53/hour
