Job Description: As an essential member of our client's dynamic team, Staff RNs play a pivotal role in delivering top-tier short-term rehabilitation care directly within patients' homes. Collaborating seamlessly with a diverse field team that includes RNs, LPNs, supervisors, and other healthcare providers, RNs bring care to patients in their most comfortable environments, ensuring quality, comfort-focused care and providing valuable educational support.
Responsibilities:
- Administer care in strict alignment with our client's policies, goals, objectives, philosophy, and Nursing Practice Standards.
- Execute hands-on patient care, encompassing responsibilities such as wound care, IV and catheter management, and medication administration.
- Oversee case-related duties, including scheduling, assessments, referrals, and meticulous documentation.
- Evaluate patients comprehensively, considering physical, psychological, and social needs, as well as environmental factors, to devise effective care plans.
- Conduct travel to patients' homes for seamless care delivery.
- Perform various types of patient visits, including initial assessments and follow-up visits.
- Provide regular reports to regional leadership.
- Actively engage in regional team activities, which may involve mentoring selected staff during orientation and offering constructive performance feedback.
- Thoroughly document all services rendered, adhering to government regulations/requirements, as well as our client's healthcare policies, practices, and procedures.
- Maintain accurate written records, including clinical and progress notes, and foster verbal communication with team members and management to ensure optimal patient/client care.
Salary Information:
- Base Salary: starting at $97,100
- Additional Compensation: $750 per year of clinical experience
- Bachelor's Degree Bonus: $1,700
- Nursing Certification Bonus (e.g., wound care, pediatrics, psychiatrics, etc.): $1,300
- Master's Degree Bonus: $1,950
- Eligibility for a 3-4% annual salary increase
Union Nurse Information:
- All field nurses are proud members of the UFT union (United Federation of Teachers - Federation of Nurses Chapter).
- Per the UFT contract, union dues are structured as follows:1st check of the month: 0.85% of gross earnings plus $20.25
2nd check of the month: 0.85% of gross earnings plus $20.25, with any remainder deducted from this check if the monthly total is less than $63.81.
- Candidates must either be existing UFT union members or not affiliated with another union.
- Membership in another union disqualifies candidates from consideration for this position.
