The Beacon of LIFE center was established to deliver PACE (Program of All-Inclusive Care for the Elderly) services. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

We are currently looking for Registered Nurse - Home Care to join our team! Full-Time position include a full benefits package including Medical, Dental, Vision Insurance, PTO, 401k.

Position Summary:

Coordinates nursing care for participants 24/7 in the center and/or at the participants’ homes. Ensures the clinic operates on a daily basis in collaboration with the Director of Nursing and physician. Delegates appropriately to Center and Clinic Nursing Assistants. Provide care in the assessment room as needed including medication administration. Ensures medical equipment is available and in working order on a daily basis. Ensures the clinic and assessment room are stocked appropriately to meet regulations and participant needs.

Basic Qualifications:

Education:

Bachelor of Science in Nursing Degree preferred. Professional Certification preferred.

Experience:

• Minimum 3 years’ experience working with the frail and elderly in acute, long term care, ambulatory or community-based setting.
• Experience with paperless documentation preferred.

Licensure/Certification:

New Jersey RN License

BLS Certification

Valid New Jersey driver’s license and proof of insurance

Pay: $41.00 - $52.00 per hour
