NTSOC, a nonprofit organization, serves southern Colorado families of children and young adults with disabilities by providing compassionate quality home care, case management, CNA training, and outpatient therapy. Job Summary: Responsible for providing client care in the field with accurate documentation.
Salary Range: $72,800 - $85,000 based on experience

Essential Functions:
1. Provides comprehensive assessment of patients to determine eligibility and continued eligibility.
2. Reports significant findings to the appropriate physician.
3. Submits documentation in a timely, complete and accurate manner.
4. Implements physician’s orders in a timely, complete and accurate manner.
5. Performs comprehensive coordination of care and case management for each assigned patient.
6. Plans, coordinates and implements the patient’s plan of care in compliance with Agency and governmental regulations.
7. Performs patient care visits in accordance with Agency productivity requirements. Provides patient care delivery in a manner that enhances the overall mission, vision, philosophy, goals and objectives of the Agency. Creates and maintains accurate field charts to facilitate continuity care.
8. Participates in a minimum of 75% of required meetings.
9. Acts as a consultant to other Agency staff.
10. Complies with all local, state, and federal laws.
