Responsibilities:

• Monitoring the patients’ progress based on the plan;
• Ensuring plan is discussed with the patient and their family, and completed in a timely manner;
• Revising the plan on an ongoing basis based on the patients’ conditions and evaluation of progress;
• Ensuring plan shows multidisciplinary planning, consultation, and education;
• Ensuring effective development and completion of discharge plan including discharge barriers and patient education.

Documents required:

• Must be actively licensed in the state in California as a Registered Nurse
• CPR or BLS Certification,
• Physical Test and a TB Test (taken within one year)
• Driver's License or State ID
• 6 months experience working as a RN
• COVID Vaccination and Booster Shot
