Role:

The Continuous Care Registered Nurse (RN) will provide and arrange for Continuous Care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Continuous Care RN makes nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.

Supervisory Responsibilities:

The Continuous Care RN is responsible for supervising LPNs and CNAs.

Experience Requirements

The Continuous Care RN must have at least one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred.

Education Requirements:

The Continuous Care RN must have at least a degree in Nursing from an accredited nursing program.

A BSN degree is preferred.

Required Certificates and/or Licenses:

The Continuous Care RN must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice; or licensure from compact state. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus.

Essential Duties and Responsibilities:

  • Explains hospice Continuous Care philosophy, goals, and services to patients and families to ensure their understanding of Capital Caring Health’s program.
  • Initiates appropriate referrals to other disciplines and/or community.
  • Initiates preventive and rehabilitative nursing procedures to maximize the quality of life.
  • Implements approved standing medical orders appropriately and effectively.
  • Teaches and supervises the teaching of family/household members/persons who are responsible for assisting the patient with his/her nursing or personal care needs to empower them to meet the patient’s needs.
  • In collaboration with the Continuous Care leadership, conducts orientation, teaching, mentoring, and training or supervision of the orientation and training of Hospice employees and volunteers assigned to the clinical service.
  • Functions as a member of the Continuous Care team by demonstrating flexibility in work schedules and shared responsibility for delivery of care.
  • Assumes responsibility for own professional growth and development in order to improve, maintain and develop new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc.
  • Participates in quality improvement activities, QUAPI program and hospice sponsored in-service training. Serves on committees or teams as approved by the clinical manager.

Organizational/Regulatory Compliance:

  • Uses the nursing process to meet the needs of the patient/family. Develops a plan of care with the patient/family and the other members of the interdisciplinary team.
  • Initiates the patient’s plan of care in accordance with nursing needs specified in the approved plan of treatment and in collaboration with patient/family, primary physician and other members of the interdisciplinary team. Identifies problems, goals and interventions.
  • Prepares and coordinates clinical notes on nursing services delivered in order to document care into the medical record.
  • Documents care provided into the medical record, including clinical information, nursing services delivered, and communications and physicians orders in a timely manner using appropriate forms/formats according to Capital Caring Health Policies/procedures.
  • Delivers, or supervises the delivery of, skilled nursing services as outlined in the plan of care with focus on palliative, preventive and rehabilitative nursing care and according to Hospice policies/procedures.

Technological Skills:

The ability to learn and efficiently use patient health information systems and demonstrate proficiency in Microsoft Office and internet browsers.

Reasoning Ability:

The ability to apply common sense understanding to carry out instructions furnished in written or oral form and deal with problems involving several concrete variables in standardized situations.

Work Environment:

Works in a business office with computers, printers, copy/fax machines and telephones. The noise level in the work environment is usually light to moderate. The environmental conditions are that of what is to be expected in a temperature-controlled building. Occasionally required to drive to alternate work locations.
