As part of Home Care and Hospice's quality improvement program, this position is responsible for reviewing nursing documentation practices and providing feedback to obtain more complete documentation that will accurately reflect the patient's clinical presentation and the individualized plan of care. The Clinical Documentation Specialist will complete concurrent chart reviews on admissions, communicate with the clinicians and coding specialists, and analyze data. The Clinical Documentation Specialist will also prepare and present clinician education sessions to improve documentation practices.
Position Qualifications:
Minimum Education Must be a Registered Nurse with a current license in the State of Indiana and a Bachelor of Science degree in Nursing.
Minimum Experience 1 to 2 years' experience in Hospice or Home Care.
Preferred Experience Previous Plan of care review experience.
Certifications Preferred Oasis, CHPN
