POSITION SUMMARY
The Primary Nurse coordinates the nursing plan of care, provides direct nursing care, and instructs/supervises the LPN and Hospice Aide staff in the delivery of nursing care to clients.
Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs. Caseloads above this target will be co-managed or additional support will be offered. In the event the assigned caseload falls below this target, the nurse may be asked to provide support to other team members.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation (See Components of Nursing Practice for the Registered Nurse)
- Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens
- Record and report the results of the nursing assessment
- Plan, initiate, deliver, and evaluate appropriate nursing acts
- Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen
- Collaborate with other healthcare providers to develop the appropriate patient and family-centered nursing plan of care for the patient and the family
- Ensure coordinated implementation of the nursing plan of care
- Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen
- Provide teaching and counseling about the patient's health, disease progression, and symptom management
- Report and record the plan for care, nursing care given, and the patient's response to that care
- Supervise, teach, and evaluate those who perform or are preparing to perform nursing functions
- Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly (by unlicensed staff, patients and families)

- Must be willing and able to care for patients with infectious disease processes unless approved accommodations
- Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation

MINIMUM QUALIFICATIONS:
Experience:
- Two years of clinical experience preferred
Licensures & Certifications: (if applicable)
- Valid NC RN license
- Valid Driver's License and proof of automobile insurance
SUMMARY OF PHYSICAL REQUIREMENTS:
- Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneeling
- Flexion/Extension of arms/wrist
- Hand dexterity to include gripping, pushing/pulling and writing
- Lifting/carrying of 50 lbs. often
OTHER DUTIES
- Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
- Participate in department and staff meeting as required
- Participate in ongoing education and training offered by the agency and/or required by nursing supervisor as part of individual education plan of RN
- Comply with company policies, procedures, guidelines, and Mission Statement
- Practice safety and infection control methods
- Assist in providing 24 hour on-call services, including coverage of at least one holiday per year
- Assess for the need of other disciplines
- Perform other but similar duties as assigned
