We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team in Columbus, OH. As a Case Manager, you will play a crucial role in coordinating and managing the care of patients, ensuring they receive the highest quality of healthcare services. This position offers a Monday-Friday schedule, providing a work-life balance for the successful candidate.
Responsibilities:
Patient Assessment and Care Planning:
- Conduct comprehensive assessments of patients' health status and needs.
- Collaborate with healthcare providers to develop individualized care plans.
- Monitor and evaluate patients' progress toward established goals.
Care Coordination:
- Coordinate with physicians, specialists, and other healthcare professionals to ensure seamless care delivery.
- Advocate for patients and facilitate communication between various healthcare team members.
- Identify and address barriers to effective healthcare delivery.
Patient Education:
- Educate patients and their families about their health conditions, treatment plans, and self-care strategies.
- Provide information on community resources and support services.
Utilization Management:
- Ensure appropriate utilization of healthcare resources.
- Collaborate with insurance providers to obtain necessary authorizations for medical services.
Documentation and Reporting:
- Maintain accurate and up-to-date patient records.
- Generate reports on patient outcomes and quality improvement initiatives.
Collaborative Teamwork:
- Work collaboratively with interdisciplinary healthcare teams to enhance patient care.
- Participate in regular case conferences and team meetings.
Requirements:
- Active Registered Nurse (RN) license in the state of Ohio.
- Bachelor of Science in Nursing (BSN) preferred.
- Proven experience in case management or a related healthcare field.
- Strong communication and interpersonal skills.
- Knowledge of healthcare regulations and reimbursement processes.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
