SalaryTitle: RN Home Health Case Manager
Location: Remote Illinois (60148, 60631, 60630, 60646)
Contract Duration: 3+ Month’s Contract (with Possibility of Extension)
Shift Requirements: 8:00 AM to 5:00 PM (5 days a week 40/hr a week 8/hr a Days).
Duties:
Patients typically have multiple comorbidities and can include (but not limited to): CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc.
Experience & Education:
Wound care, including wound vacs, pleural drains, ostomy care; peripheral blood draws, central line care, IV administration through multiple devices including elastomeric and IV pumps, excellent assessment skills, experience with computer documentation
Job Type: Contract
Pay: $70.00 - $88.00 per hour
Expected hours: 40 per week
