As the Admissions Nurse, you will have the opportunity to collaborate with our Director of Clinical Services and clinical teams to contribute to that mission. These passionate team members will depend on you to perform initial terminal diagnosis impact assessments on the patient's physical, functional, psychosocial, and environmental needs and daily living.
On an average day, you will
  • Determine the scope and frequency of services needed based on acuity and patient/family needs of initial assessment and re-assessments
  • Assess cultural and spiritual implications, verbal and non-verbal communication patterns, and risk for pathological grief
  • Implement individualized plans of care and recommend revisions to plans as necessary.
  • Manage patient discomfort and provide symptom relief
  • Consult with and educate patient and family regarding disease process, self-care techniques, end-of-life care, and the process for dealing with issues of ethical concern
  • And perform additional, related duties as needed

Qualifications
Desired Skills/Qualifications
  • Must have an active RN license
  • At least 2 years of nursing experience, preferably in hospice
  • Previous admissions experience preferred
  • Have a valid driver's license, auto insurance, and reliable transportation for the purpose of traveling to our patients
  • Strong verbal and written communication skills
  • Ability to complete patient charting on a daily basis
  • A passion for providing care
  • A desire to take the next big step in your career
