ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Assures the provision of Nursing and Health Related services for Residential consumers according to all applicable DDS, Mass Health, MRC, DPH and other relevant regulations.

  1. Health Care Coordination
  2. Assess individual’s medical regimes as assigned to ensure that all individuals are referred for and receive health services, which will maximize health and safety.

 

  1. Ensures that individuals are referred for and receive services which assist with achieving set goals and maximize physical and emotional well-being.

 

  1. Maintenance and auditing of medical charts, providing recommendations for medical follow-up to Residential Providers and Parents/Guardians.

 

  1. Conduct nursing assessments and monitor overall well-being of each individual as assigned.

 

  1. Review Health Care Provider’s orders for accuracy.

 

  1. Adheres to DPH regulations for medication storage, administration and destruction.

 

  1. Responsible for nursing specific medication administration and clinical procedures as assigned.

 

  1. Complete detailed weekly medical updates as assigned and distribute to assigned parties via e-mail.

 

  1. Assist in obtaining reports and approval of treatment plans from the individuals’ Health Care Providers.

 

  1. Participate in Agency Interdisciplinary Team Meetings.

 

  1. Monitor the general health status of all individuals assigned on a day-to-day basis, and attend doctor’s appointments when needed.

 

  1. Attend all mandatory meetings and maintain required licensures.

 

  1. Adhere to DPH, DMR, MRC commonwealth licensure, LifeStream policies.

 

  1. Maintain open and effective communication with DDS/MRC Service Coordinators, peers, supervisors, parents, guardians and community health care providers.

 

  1. Implements a health care delivery model, which meets the diverse needs of the individuals, served with an emphasis on consumer empowerment and use of community resources.

 

  1. Maintains respectful communication with all individuals served.

 

  1. Reports all suspicions of abuse or neglect to DPPC as a mandated reporter.

 

  1. Ensure compliance with all dietary orders and restrictions.

 

  1. Coordinate hospital admissions and discharges as needed.

 

  1. Conduct Annual Medical Assessments for individuals within assigned area.

 

  1. Review all Health Care Provider Encounter Forms for individuals within assigned area.
  2. Develop medical protocols in partnership with Health Care Providers, and update as needed.

 

  1. Acts as LifeStream’s Medication Administration Program Consultant and educates staff in medication administration polices as indicated by the Department of Public Health. 

 

  1. Participate in Nursing On Call rotation as a MAP Consultant and medical support to staff and individuals.

 

 

 

  1. Training
  2. Identify areas of clinical development needed for program staff and coordinate trainings to address any needs identified.
  3. Participate in the training of new LPN hires consistent with Nurse Mentorship Program.
  4. Conduct medically related orientation trainings as necessary.
  5. Conduct RN required trainings consistent with MAP.
  6. Act as a resource for LPN’s working in programs and provide education and support as needed. 

 

  1. Quality Assurance
  2. Attend all required meetings and maintain required licensures.
  3. Coordinate health care needs of individuals served and assure that delivery of these services occurs in a timely fashion.
  4. Review medication administration systems on a monthly basis for assigned programs with the development of an action plan, to be completed with Residential Coordinator in a timely manner.
  5. Conduct monthly site visits to homes within assigned area and provide feedback regarding Medication Administration/components of the medication closet as outlined in MAP Technical Assistance Tool, Protocols, Training Competences, and Clinical Progress Notes.

 

  1. Administrative
  2. Ensure that all required medical documentation is completed within a timely fashion and according to established guidelines and reflect the progress and interest of each individual served.
  3. Maintain open and effective communication with DDS/MRC Service Coordinators, parents, guardians, employees, other managers, vendors, and community.
  4. Effective communication of all programs needs reported to Residential Coordinator and Director of Nursing in a timely manner.
  5. Report any suspicions of abuse or neglect to DPPC as a mandated reporter.
  6. Attend all required meetings.
  7. Perform all other duties as assigned by the Director of Nursing.

 

 

QUALIFICATIONS AND REQUIREMENTS:

  • Maintenance of an acceptable CORI
  • Valid Driver’s license
  • Negative drug screen
  • Use of personal vehicle for work purposes
  • Initial physical exam with mantoux testing. Mantoux screening annually thereafter.
  • Commitment to the philosophy of community inclusion
  • Maintain flexibility with weekly scheduled hours in order to accommodate program needs.

 

EDUCATION and/or EXPERIENCE:  

  • Registered Nurse with current licensure in Commonwealth of Massachusetts.
  • Demonstrated history of quality work performance and teamwork in previous positions
  • One year supervisory experience or a minimum of 5 years’ experience in Human Services preferred.
