Job Summary

Works in collaboration with all members of the health care team to develop and maintain initiatives to assess and coordinate chronic disease management of patients with pulmonary diseases. The Respiratory Care Patient Navigator identifies and coordinates post-discharge services, education and follows-up on needs across the continuum of care, during transition of care.

Minimum Qualifications

Minimum Education: Graduate of a 2 year Associate Degree registered through the National Board for Respiratory Therapy or related field
Minimum Years of Experience: 3 Years of critical care experience
Substitutable Education & Experience: None.
Required Certifications/Licensure: Licensed by the Board of Medical Examiners
Required Training: Completion of COPD Educator Course within 6 AMA accredited program for Respiratory Care.

  • Develops and implements patient-centered care plans and discharge pathways to support self-management of pulmonary disease to prevent readmissions.
Duties & Responsibilities
  • Identifies and coordinates post-discharge services, education, and resources for COPD Transition of Care Program.
  • Serves as clinical resource for assigned patient populations as needed.
  • Participates and leads multidisciplinary teams to support organizational goals for quality initiatives to include length of stay and readmissions.
  • Collects and analyzes data for expected outcomes and opportunities for improvement.
  • Develops collaborative working relationships with medical staff to provide transitional needs of the patient and family.
  • Develops policies and procedures as needed for the transitional care program.
  • Researches, develops and implements special projects as identified to promote best practices.
  • Serves as clinical resource to determine appropriate resources for inpatient units, emergency department, ancillary services and the community.
  • Performs all other duties as assigned
